PALLIATIVE CARE FOR ADVANCED NON-MALIGNANT DISEASES (PCAND)
Palliative Care for Advanced Non-malignant Diseases (PCAND) is a community of practice of specialist palliative care providers who are providing care to patients with advanced non-malignant diseases in the ambulatory setting. The ambulatory setting presents an opportunity to engage specialist palliative care early, especially in the face of the unpredictable trajectories that characterize non-malignant diseases. This community of practice seeks to support its members clinically, academically, and educationally.
LEARNING OBJECTIVES
By engaging with the PCAND community of practice, participants should be able to:
- Identify challenges and barriers to optimal care in patients with advanced non-malignant diseases in the ambulatory setting.
- Problem-solve collaboratively, drawing on diverse experiences within the community of practice to enhance clinical decision-making and patient care outcomes.
- Assess strategies for enhancing the quality of care provided to people with advanced non-malignant diseases in the ambulatory setting by means of quality improvement initiatives or research projects.
